Thank you for registering for the 4-Hour Dementia Training. Please enter your full name (first & last name) and the name of your employer at the beginning of each module. There will be a competency quiz after each module. You will need to get 100% on the quiz to advance to the next module. The introduction and all five modules must be completed. You can resume learning if you need to take a break.

After the final module, you will be prompted to receive your certificate. Please save the certificate for your records. Also, please print a copy for your current employer. The certificate must be added to your personnel file / training record.  This training is portable and if you change employers, you can take this to your next employer.
